Registration Fee

A one-time, non-refundable fee of $25 per family is required at time of enrollment. The registration fee will be automatically charged to your card upon enrollment.

As a special for new students only, the registration fee is currently being waived.



When enrolling in the school-year program, please remember that your child is registered for the full school season, August through May.


Consent and Release of Liability

As the parent or legal guardian of the student, I consent to my child's participation of Stars Ballroom, LLC dance training, rehearsals, and performances, as well as recreation activities to enrich the student's dance experience. I understand that dance is a very physical activity, and there is a risk of accident or injury inherent in dance. I am also aware, and agree, that dance is a physical activity that requires hands-on correction from teacher to student to reinforce proper alignment and positioning of the body. I agree that Stars Ballroom, LLC, along with its teachers, consultants, volunteers, director, owner, and/or agents, shall not be responsible for injuries or death to be caused during my child's participation or en route to or from Stars Ballroom, LLC. I do hereby voluntarily waive and release any and all actions, claims, and demands for any damage, injury, death, or loss to person or property that may be sustained by myself, my child, and/or indirectly during the course of, or as a result of, my child's participation as set above. I hereby grant permission to the person listed as the emergency contact, my child's physician, the studio Director, or the Director's designee to take whatever action is deemed necessary for the health of the aforesaid child in case of injury, accident, or illness, until I, my spouse, or the child's legal guardian can be contacted. I further authorize the release to health care workers any medical or health care information Stars Ballroom, LLC, or the person listed above may have regarding the student, and the use of such information by health care workers in the subsequent medical care of the student. I execute a complete and unconditional release of liability to the greatest extent allowed by law by the State of Arizona.

I, the undersigned, am at least 18 years of age, and I am the student, student's parent, or student's legal guardian. I understand, by clicking the "Submit" button, I am electronically signing with the intent to document my agreement to waive liability and abide by all policies and procedures in this agreement. I further understand that my signature has the same effect as my hand-written signature.


Publicity Release

I hereby give any permission and consent for my child's image to be included in promotional material, such as marketing brochures, posters, fliers, video and website for Stars Ballroom, LLC, its programs, and productions, without compensation to the student.


Email List

Occasionally, announcements regarding discounts, new products and services, or changes to our policies, will be sent by email. You hereby agree to receive such emails, and understand that you can unsubscribe, or opt out, at any time.


Payment of Tuition

Tuition is non-refundable. Tuition shall be due and payable at time of enrollment, and continue monthly (the first day of each month) from August through May for the school-year dance program. For the six-week summer program, and any other program offered, tuition shall be due and payable at enrollment. Tuition shall be automatically charged to your card on file. All accounts are required to have a credit card on file. If a student becomes inactive, and follows proper procedures to withdraw, the card on file will no longer be charged after that. (See "Withdrawals" under studio policies.)

Temporarily, a link will be sent to pay class tuition until our upgrades are completed. Thereafter, tuition will be charged on a monthly basis per the "Payment of Tuition" paragraph above.


Late Fees/NSF

If a payment is rejected due to non-sufficient funds, a late fee of $35 will be charged to your card after five (5) days past the due date. Also, a late fee of $25 will be charged if any tuition payment due is not received within five (5) days of the due date.


A thirty-day prior written notice must be provided on the "Withdrawal" form available on the studio's website, If a thirty-day prior written notice has not been filed, the card on file will continue to be charged. Failure to attend classes does not constitute an official withdrawal. No refunds will be granted for classes not attended. However, if a thirty-day official written notice has been given, tuition may be pro-rated for the following month.


Registration fees, recital fees, and costume fees are non-refundable. Tuition is non-refundable, with the exception of serious injury/illness only. Requests for a refund must be made in writing and documentation from your physician must be submitted within 10 days of injury or illness. A refund may be granted, and will be pro-rated, for classes missed from the date the written request is received.

Dance Recital

A dance recital will be offered. Your child will enjoy showing what they have learned in dance class. It will be a great experience seeing your child perform. Recital and costume fees will apply to offset production fees. If your child performs in the recital, recital and costume fees will be charged to your card on file after obtaining your authorization.

Summer Program

A summer program is offered for continuity of training to benefit the student. The summer program is six weeks in length. However, the summer program can be pro-rated for those that cannot attend the full six-week program due to summer vacations, etc. If enrolled, the summer program also will be charged to your card on file.


Students are encouraged to attend the class(es) they are registered for on a regular basis. Students will have their attendance taken in the classroom by their teacher. All students should be in class and on time and ready to participate in class. Students should be appropriately dressed for the dance lesson they are attending. Students' hair should be pulled back away from the face. No dangling jewelry should be worn. No chewing gum, food, or drinks other than water are permitted in the studio to assist in the maintenance of the studio and particularly due to the premium maple wood flooring. Parents are asked not to observe the class except for classes in which they actively participate, as it distracts the students. However, there will be a date set aside for parents to view their children in class. Parents are expected to pick up their children promptly after class. Please do not leave your child unattended for his or her own safety.

Make-up Classes

Make-up of missed classes is encouraged! To schedule a make-up class, please email - There are no refunds for missed classes.


Stars Ballroom, LLC reserves the right to cancel a class at any time. Also, a class must have at least three (3) participants to be held, and may be cancelled if this minimum is not met. If a class is cancelled, Stars Ballroom, LLC, at its sole discretion, will choose to either schedule a make-up class, or credit the student's account.
